As we prepare for the pilot phase of our project, we are busy finalising our package of interventions to implement in various clinics. These interventions are aimed at integrating heart health care into ART programmes. Some of the proposed interventions include, placement of care coordinators in facilities and distribution of home self-monitoring equipment, such as, blood pressure and blood glucose monitors to patients.
